  • ​BCBSIL PPO Plans

  • BCBSIL Blue Choice

​Note specific coverage varies by plan, so it's always a good idea to confirm details directly with BCBSIL. 

A Few Helpful Things to Know:

​👉  No prescription or referral is needed to start PT

👉  No prior authorization is needed for most PPO plans

👉  All BCBSIL claims are submitted electronically for you on your behalf

Other Insurance Plans

My Pelvic Therapy is out-of-network with insurance plans outside of BCBSIL PPO and Blue Choice.

If you have a different insurance and you think you may have out-of-network benefitsyou may request a superbill following each visit, which you may then submit to your insurance company directly for possible reimbursement.​​

Using HSA or FSA Cards

Pelvic floor physical therapy is considered a qualified medical expense and is typically covered by HSA (health savings account) and FSA (flexible spending account) plans when care is medically necessary.

Many patients use HSA or FSA funds to pay for visits, including virtual pelvic floor physical therapy, either on their own or alongside insurance coverage for copays, deductibles, or other out-of-pocket costs. This allows you to use pre-tax dollars for care .

Self-Pay Options

Self-pay is a common option for pelvic floor physical therapy, especially for virtual care.

  • A package of 5 visits is offered at a reduced rate for further savings
